PATH (People Assisting The Homeless) is seeking candidates with experience serving vulnerable populations to join our Veteran Services SSVF Program as a Rapid Resolution Specialist.

WHAT WE DO

Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.

PATH’s SSVF Programs work to promote housing stability among very low-income Veteran families who reside in or are transitioning into permanent housing. The SSVF program focuses heavily on linking Veterans to resources provided by the Department of Housing and Urban Development (HUD) Veterans Affairs Supportive Housing (VASH) program.

WHAT IS A RAPID RESOLUTION SPECIALIST?

As part of the SSVF team, the Rapid Resolution Specialist triages all screenings for the SSVF program to evaluate client eligibility, determine potential housing options, and link veterans to SSVF and other appropriate resources. Responsibilities of the Rapid Resolution Specialist include:

+ Conducting initial assessments to establish initial eligibility and appropriateness of Rapid Resolution, SSVF or other services

+ Maintaining a resource directory and providing appropriate resources to clients

+ Directing and tracking referrals to appropriate Veteran Services departments

+ Communicating with enrolled veterans and supporting households on a weekly basis to review and evaluate progress and provide support

+ Providing on-going case management supports including:

+ Assistance with identifying appropriate housing opportunities

+ Mediation and advocacy support with landlords

+ Initiating and processing TFA requests

+ Providing information, referrals, linkage, and advocacy for all identified needs

WHAT YOU BRING

We’re looking for candidates that:

+ Have experience working with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues

+ Demonstrate knowledge of or experience with advanced, evidence-based case management techniques such as Harm Reduction strategies, non-violent crisis intervention techniques, and Motivational Interviewing

+ Maintain and execute confidential information according to HIPAA standards

+ Demonstrate strong skills with technology as a work tool including proficiency with Microsoft Office

+ Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs

+ Work just as well independently as they do on a team

+ Exercise mature judgement, and are highly motivated, self-starting and proactive

+ Are excellent at communicating, whether in writing or verbally

+ Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

MANDATORY REQUIREMENTS

For this role the Rapid Resolution Specialist must:

+ Be able and willing to work flexible hours which may include evenings or weekends

+ Have employment eligibility verification

+ Have or be able and willing to obtain CPR/First Aid training

+ Have reliable transportation and:

+ A valid driver’s license

+ Proof of insurance and ownership for personal vehicles used during work duties

+ The ability to qualify for PATH insurance coverage

+ Successfully complete the following as a condition of hire:

+ Tuberculosis Test

+ Background Screening

+ Drug Test
