Job Overview The Care Coordinator is responsible for providing culturally sensitive behavioral health services focused on alcohol and other drugs issues for the youth who are in a long-term placement at Log Cabin Ranch (LCR) under the jurisdiction of San Francisco Juvenile Probation Department (JPD). LCR is a residential program for youth who have been adjudicated delinquent by the Juvenile Courts. The Care Coordinator conducts screening, assessment, and intake of each resident of LCR, and she/he provides individual and group counseling to youth utilizing evidence-based models in coordination with LCR staff, DPH personnel, and other community partners. The Care Coordinator is responsible for providing AOD workshops to families of LCR residents and JPD staff periodically. Key Responsibilities Screening, Assessment & Plan Development Responsibilities: Facilitates individual counseling sessions and case management with each caseload client. Provides clinical support to assist clients in meeting their treatment goals. Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status. Treatment Setting Responsibilities: Facilitates group counseling sessions and teaches psychoeducation classes on various alcohol and other drugs topics. Performs crisis intervention and communicates with treatment team as unforeseen situations arise. Attend required trainings and meetings. Willingness to work flexible hours. Documentation Responsibilities: Maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client. Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner. Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements. Collaboration & Outreach Responsibilities: Under supervision of Program Manager, attends inter-agency meetings regarding coordination of services for LCR residents; maintain positive and collaborative relationships with personnel from LCR, JPD, SFUSD, DPH, and other community partners. Facilitates periodical workshops on alcohol and other drug issues to family members of LCR residents, Provides consultation to LCR staff on alcohol and drug issues as needed, and does presentations on the treatment program to Juvenile Courts and community stakeholders. Other duties as assigned. Education and Knowledge, Skills and Abilities Required: Current and valid Alcohol and Drug Counseling Registration or Certification recognized by DHCS (e.g. CAADE) Experience providing group counseling -Experience working in residential settings Experience working with criminal justice system Desired: Experience using Evidence Based Models including Seeking Safety, Motivational Interviewing, and MET/CBT-5 (Motivational Enhancement Therapy/Cognitive Behavioral Therapy), Dialectical Behavioral Therapy Background Clearance Required: Must not be on active parole or probation Must be able to pass Fingerprinting and Background Checks to work with minors Knowledge Required: Culturally competent and able to work with a diverse population Strong proficiency with Microsoft Office applications, specifically Word, Outlook and internet applications Valid CA Driver's license First Aid Certified within 30 days of employment CPR Certified within 30 days of employment Desired: Knowledge of co-occurring disorders and trauma-informed treatment Knowledge of clinical documentation (treatment plans, progress notes etc.) Other jobs you may like |
